Generic sildenafil tablet as low as $3/tablet and generic tadalafil as low as $5/tablet, each on a 12-month supply (provider-published, verify current)
Check price →Read review ↓Trimix is a compounded injection for erectile dysfunction that combines three drugs: papaverine, phentolamine and alprostadil. It is injected into the side of the penis and causes an erection within minutes, without relying on the pathway that pills like Viagra use. Urologists often turn to it when pills do not work or cannot be taken.
Trimix is not FDA-approved as a combination. The AUA guideline says only alprostadil is FDA-approved for injection into the penis, sold under brand names such as Caverject and Edex. Papaverine and phentolamine have been used for ED for decades, but neither is FDA-approved for this use. Trimix is mixed by a compounding pharmacy for one patient, and the dose must be set in a clinic first.
This guide is educational and is not medical advice. It is for adults 18 and older. Injections require a prescription and hands-on training from a clinician. ED Samples is reader-supported and may earn a commission from partners below; that never changes the facts we report.
The short version
- Trimix = papaverine + phentolamine + alprostadil, injected into the penis. It is compounded, not FDA-approved as a combination.
- Only alprostadil (Caverject, Edex) is FDA-approved for penile injection, per the AUA guideline.
- The AUA says men considering injections should first have an in-office test injection, so a clinician can find a safe dose and teach the technique.
- Priapism, an erection lasting more than 4 hours, is the most serious risk and a medical emergency. The AUA reports the lowest rates (mean 1.8%) with alprostadil alone.
- Other side effects include penile pain, bruising and scarring (fibrosis), reported in 4.5% to 13% of men across injection studies.
- Trimix vs Viagra: pills are first-line and simpler; injections often work when pills fail. HealthyMale lists Tri-Mix and Quad-Mix with a physician consult.
| Option | How it is taken | FDA status | Main risks |
|---|---|---|---|

| Viagra / sildenafil, Cialis / tadalafil | Oral pill | FDA-approved (brand and generic) | Headache, flushing; never with nitrates |
HealthyMale | Phone consult, then pharmacy ships | Generic pills FDA-approved; Tri-Mix/Quad-Mix compounded | Same as the medicine prescribed |
| Video visit, e-prescription to your pharmacy | Pharmacy-filled | Same as the medicine prescribed | |
| Nu Image Medical | Online intake, ships if prescribed | Generic tablets FDA-approved; blends compounded | Same as the medicine prescribed |

01 · Telehealth pharmacy that lists compounded Tri-Mix and Quad-Mix
Lists Tri-Mix
HealthyMale
A long-established US men's-health pharmacy and telehealth service offering the broadest mix of generic and brand ED medications without a forced subscription.
Clinical oversight: Strong, verifiable accreditation (VIPPS pharmacies, LegitScript, HIPAA) and unusually transparent itemized pricing; what we could not independently confirm is real-time stock and any state-by-state variation, so verify at checkout.
HealthyMale is one of the longest-operating men's-health telehealth services in this guide, with the company reporting operations since 1998 and pairing US-licensed physicians with VIPPS-certified pharmacies. For erectile dysfunction it has the widest published catalog of the approved partners: generic sildenafil (generic Viagra) and tadalafil (generic Cialis), plus brand Viagra and Cialis, vardenafil, avanafil (Stendra), and specialty compounded injectables (Tri-Mix/Quad-Mix). Access runs through a phone consultation with a licensed physician, and a prescription is issued at the clinician's discretion.
On price, HealthyMale is unusually transparent for this category. Generic sildenafil is advertised starting around $20, generic tadalafil from around $20, brand Viagra around $129, brand Cialis around $104, and daily tadalafil around $198, with the site noting a free doctor consultation and free prescription with purchase and free shipping over $50. Treat all of these as ‘starting at’ figures and confirm at checkout.
For trust signals, the site displays LegitScript verification, HIPAA compliance, and VIPPS-certified pharmacies, the kind of credentials worth prioritizing in a Your-Money-or-Your-Life category. As with every provider here, this is a path to a legitimate consultation, not a way around one.
- Status
- Approved partner
- Rx options
- Generic + brand sildenafil & tadalafil; vardenafil, avanafil, Tri-Mix/Quad-Mix
- Consult flow
- Phone consultation with US-licensed physician
- Pricing model
- À-la-carte; no forced subscription
- Delivery
- Discreet home delivery; free shipping over $50
- Credentials
- VIPPS pharmacies, LegitScript, HIPAA
- Operating since
- 1998 (Tempe, AZ)
What we like
- Widest published ED menu: generic and brand sildenafil and tadalafil plus vardenafil, avanafil, and injectables
- Transparent, itemized pricing (generic sildenafil/tadalafil from ~$20)
- À-la-carte ordering with no forced subscription
- VIPPS-certified pharmacies plus LegitScript and HIPAA signals; free doctor consult advertised
Worth noting
- Consult is by phone rather than live video
- Brand-name medications (Viagra ~$129, Cialis ~$104) are priced well above generics
- Prices shown as 'starting at', verify at checkout
Who should buy it: Adults 18+ who want the broadest range of generic and brand ED options, transparent pricing, and the flexibility to order à-la-carte after a phone consult rather than being placed on auto-ship.
What we don't like: The consult is phone-based rather than on-demand video, and brand-name pricing is high relative to the inexpensive generics, most people will want to ask specifically about generic sildenafil or tadalafil.
Bottom line: Our top approved pick for most people: HealthyMale publishes real, itemized pricing across generic and brand sildenafil and tadalafil, adds less common options like vardenafil, avanafil, and specialty injectables, and lets you order à-la-carte after a phone consult rather than locking you into auto-ship.
02 · Video visit that can prescribe alprostadil or pills to your pharmacy
Best for Fast Video ConsultsDrHouse
An on-demand telehealth platform where a licensed physician can join by video within about 15 minutes and e-prescribe ED medication to the pharmacy of your choice.
Clinical oversight: Clear credentialing (LegitScript, HIPAA), stated 50-state availability, and a published visit price; what we could not confirm is the final medication cost, which depends on the drug and pharmacy you select.
DrHouse is a real, operating on-demand telehealth platform that is LegitScript-approved, HIPAA-compliant, and available in all 50 states, with insurance accepted for visits. For erectile dysfunction it offers on-demand video visits, the company states a provider can join in roughly 15 minutes, in which a licensed physician evaluates ED and, when clinically appropriate, prescribes sildenafil (Viagra), tadalafil (Cialis), vardenafil (Levitra), avanafil (Stendra), or alprostadil. The prescription is at the provider's discretion, and DrHouse does not prescribe controlled substances.
The consult flow is the standout here. Rather than an asynchronous questionnaire, you get a live video visit, and the company says insurance may cover it. The ED visit is listed at $129 or your copay; the medication itself is filled at the pharmacy and priced separately, so your total cost depends on the drug and pharmacy you choose.
If you want a structured, all-in-one subscription with medication included in one price, DrHouse's model will feel less packaged than the big DTC brands. But for a fast, insurance-eligible clinical visit with flexibility on where you fill, it's a strong, credentialed option.

- Rx options
- Sildenafil, tadalafil, vardenafil, avanafil, alprostadil (no controlled substances)
- Consult flow
- On-demand live video visit (~15 min)
- Pricing model
- Per-visit; medication priced separately at pharmacy
- Availability
- All 50 states; insurance accepted for visits
- Delivery
- E-prescription to pharmacy of choice
- Credentials
- LegitScript-approved, HIPAA-compliant
What we like
- Fast on-demand live video visit (provider in ~15 min) across all 50 states
- Clear visit price ($129 or copay); insurance may cover the visit
- Wide prescription range including vardenafil, avanafil, and alprostadil
- E-prescribes to your chosen pharmacy, so you can shop medication prices
Worth noting
- Medication is priced separately, so total cost isn't a single bundled number
- No medication-included subscription like the big DTC brands
- Does not prescribe controlled substances (not an issue for standard ED meds)
Who should buy it: Adults 18+ who want a fast, live video consult with a licensed physician, the option to apply insurance to the visit, and the freedom to fill their prescription at any pharmacy.
What we don't like: Because medication is filled and priced separately, you have to estimate drug cost yourself, and there's no all-in subscription price to compare at a glance.
Bottom line: The best approved pick if speed and the chance to use insurance matter to you: DrHouse offers fast live video visits in all 50 states, publishes a clear visit price, and lets you fill the prescription at any pharmacy, just note medication is priced separately.
03 · FDA-approved generic pills to try before injections
Lowest Per-Tablet PriceNu Image Medical ED Tablets
A telehealth clinic founded in 2004, online since 2008 and available in all 50 states, that lists FDA-approved generic sildenafil and tadalafil tablets alongside compounded combination options.
Clinical oversight: Per-tablet prices published on the provider's own men's sexual performance page, with each product labeled FDA-approved or compounded. Every order is subject to a medical consultation and ships only if a licensed U.S. provider prescribes it.
Nu Image Medical is a telehealth clinic that says it was founded in 2004, has operated online since 2008 and is available in all 50 states. Its men's sexual performance line lists four options (all provider-published): a generic sildenafil tablet as low as $3 per tablet and a generic tadalafil tablet as low as $5 per tablet, both FDA-approved and both priced on a 12-month supply; a compounded sildenafil plus tadalafil rapid-dissolve tablet as low as $4 per tablet on a 5-month supply; and the compounded Mt. Everest program (tadalafil, oxytocin and PT-141) from $4 per tablet on a 5-month supply.
You share your goals online, a licensed U.S. provider reviews them, and medication ships discreetly only if it is prescribed.
- Status
- Approved partner
- Published price
- Sildenafil from $3/tablet, tadalafil from $5/tablet, 12-month supply (verify)
- Consult
- Online intake reviewed by a licensed U.S. provider
- Operating since
- 2004 (online since 2008)
What we like
- Lowest published per-tablet price we track on FDA-approved generics
- Each product labeled FDA-approved or compounded
- Available in all 50 states
Worth noting
- Lowest prices require a 12-month supply
- Combination and Mt. Everest products are compounded, not FDA-approved
Who should buy it: Adults 18+ who want an FDA-approved generic tablet at a low per-tablet price and are comfortable buying a multi-month supply.
What we don't like: The lowest prices depend on a 12-month supply, so the up-front cost is higher than a one-month order, and the combination products are compounded rather than FDA-approved.
Bottom line: If you want the FDA-approved generic tablet and are fine buying a longer supply, Nu Image lists the lowest per-tablet prices we track: sildenafil as low as $3 and tadalafil as low as $5. The low figures are tied to a 12-month supply, so check the up-front total before you pay.
What is Trimix?
Trimix is a compounded ED injection made of three drugs: papaverine, phentolamine and alprostadil. Each relaxes the smooth muscle and blood vessels in the penis in a different way, so together they produce an erection at lower doses of each drug. The AUA guideline lists this exact combination as a common intracavernosal injection, meaning a shot into the corpus cavernosum, the spongy tissue along the side of the penis.
Related blends exist. Bimix usually means papaverine plus phentolamine. Quad-Mix adds a fourth drug, often atropine, which the AUA guideline also names. A compounding pharmacy prepares each vial to a prescriber's formula, so strength varies from one prescription to the next.
Is Trimix FDA-approved?
No. Trimix is a compounded medication, and the combination is not FDA-approved. The AUA guideline states that only alprostadil is FDA-approved in the U.S. for injection into the penis, and that the non-prostaglandin agents (papaverine and phentolamine) are not formally FDA-approved for ED, even though they have been used successfully for decades.
Compounding is legal when a licensed prescriber orders it for a specific patient. What you give up is FDA review of that exact product for safety, effectiveness and manufacturing quality. If you prefer an FDA-approved injection, ask about alprostadil alone, sold as Caverject or Edex. The FDA explains the trade-offs in its compounding questions and answers.
How Trimix injections work
Trimix acts directly on the penis, so it does not need the nitric oxide signal that ED pills amplify. Papaverine and alprostadil relax smooth muscle and open the arteries, and phentolamine blocks the signals that keep blood vessels narrow. Blood fills the erectile tissue and the erection follows, usually within minutes.
That direct action is why injections often work for men whose pills failed, including men after prostate surgery or with diabetes. The AUA guideline reports that 53.7% to 100% of men in injection studies achieved an erection good enough for intercourse, with no marked difference across the medications or combinations studied.
The in-office test injection
Your first Trimix injection should happen in a clinic, not at home. The AUA guideline says men considering injections should first have an in-office injection test. MedlinePlus says the same for alprostadil: the doctor gives the first dose in the office to find the right dose, and you are trained before using it at home.
The test dose does three jobs. It confirms the drug works for you. It finds the lowest effective dose, which the AUA calls critical to reducing priapism risk. And it lets the clinician teach the technique: where to inject, how to rotate sides and how to apply pressure afterward. You should also leave with a written plan for what to do if an erection does not go down.
Trimix side effects
The common side effects are penile pain or aching, bruising or bleeding at the injection site, and, over time, scarring. MedlinePlus lists bleeding or bruising, pain in the penis, testicles or perineum, penile redness, dizziness and rash for alprostadil. The AUA guideline notes pain is highest with papaverine or alprostadil used alone and with papaverine plus phentolamine, and that penile fibrosis, plaque or curvature were reported in 4.5% to 13% of men across injection studies.
- Call a doctor for new lumps or hard areas in the penis, unusual curving, swelling or tenderness. These can be signs of scarring.
- Get emergency care for an erection lasting more than 4 hours, fainting, a fast heartbeat, or a needle that breaks in the penis (MedlinePlus).
Rotating injection sites, using the smallest dose that works, and not injecting more often than prescribed all lower the risk.
Priapism: the main risk
Priapism is an erection that lasts more than 4 hours, and it is the most serious risk of Trimix and other penile injections. The AUA guideline calls it the most serious adverse event of injection therapy and reports the lowest rates, a mean of 1.8%, in studies using alprostadil. Untreated priapism can permanently damage erectile tissue.
Never raise your dose on your own, never inject twice within the window your prescriber sets, and do not combine Trimix with ED pills unless your clinician has told you to.
Trimix vs Viagra
Viagra (sildenafil) is a pill and the usual first choice; Trimix is an injection that often works when pills do not. Pills are easier and FDA-approved, and they need sexual stimulation to work. Trimix acts directly on penile tissue, so it can produce an erection in men whose nerves or blood vessels do not respond to pills.
- Ease: pills win. Injections need training and a needle each time.
- Who can use it: pills cannot be taken with nitrates. The AUA lists injections as an option for men with contraindications to pills or for whom pills are inadequate.
- Regulation: sildenafil is FDA-approved as brand and generic. Trimix is compounded.
- Main risks: pills cause headache and flushing. Injections carry priapism, pain and scarring risk.
For the pills themselves, see sildenafil vs. tadalafil and sildenafil side effects.
Who is a candidate for Trimix?
Trimix is usually considered for men whose ED did not respond to pills, who cannot take pills, or who prefer not to. The AUA guideline says injections work in the general ED population and in men with diabetes, cardiovascular risk factors, spinal cord injury and after prostate removal.
A clinician will want to know about blood thinners, bleeding disorders, sickle cell disease or other conditions that raise priapism risk, Peyronie's disease, and whether you can see and reach the injection site. If you are not ready for needles, the AUA also lists urethral alprostadil and vacuum erection devices as options.
How to get Trimix
You need a prescription, and the first dose should be supervised. Most men start with a urologist, who does the test injection and orders Trimix from a compounding pharmacy. Some telehealth pharmacies also list it. HealthyMale's ED page lists Tri-Mix and Quad-Mix compounds alongside generic pills, with a phone consultation with a licensed physician (checked October 30, 2026). Ask any provider how the in-office test dose and training will be handled before you order.
Insurance rarely covers compounded drugs, so ask for the full price, the vial strength and how many doses it holds. Ask how the vials should be stored and when they expire. Never buy injectable ED drugs from a seller that does not require a prescription.

Questions, answered
What is Trimix?
Trimix is a compounded ED injection that combines papaverine, phentolamine and alprostadil. It is injected into the side of the penis and produces an erection without relying on the pathway ED pills use.
Is Trimix FDA-approved?
No. Trimix is compounded and the combination is not FDA-approved. The AUA guideline says only alprostadil is FDA-approved for penile injection in the U.S., sold as Caverject and Edex.
How fast does Trimix work?
Usually within minutes, because it acts directly on the penis. Your clinician will confirm the timing and dose during the in-office test injection.
What are the side effects of Trimix?
Penile pain or aching, bruising or bleeding at the injection site, and over time possible scarring or curvature. The most serious risk is priapism, an erection lasting more than 4 hours, which needs emergency care.
Is Trimix better than Viagra?
Not better, different. Viagra is an FDA-approved pill and the usual first choice. Trimix often works when pills fail or cannot be taken, but it requires injections, training and carries a priapism risk.
Does the first Trimix shot have to be in a doctor's office?
It should be. The AUA guideline says men considering injections should first have an in-office test injection, so the clinician can find a safe dose and teach the technique.
Does Trimix hurt?
Most men describe a brief pinch from the small needle. Some feel an ache in the penis afterward. The AUA notes pain is more common with some single drugs and combinations; tell your prescriber if it persists.
What should I do if my erection does not go away?
If an erection lasts more than 4 hours, go to an emergency room. Ask your prescriber for a written plan for prolonged erections before you start.
Can I get Trimix online?
Some telehealth pharmacies list it. HealthyMale lists Tri-Mix and Quad-Mix with a phone consult. A prescription and a supervised first dose are still required, so ask how the provider handles the in-office test.
