The short version
- How long Cialis lasts: up to 36 hours after an as-needed dose, per the FDA label. That is a window for sexual stimulation to work, not a 36-hour erection.
- Cialis half-life: 17.5 hours in healthy men. Peak blood levels arrive 30 minutes to 6 hours after a dose (median 2 hours), and food does not change it.
- Common side effects (label, 10 to 20 mg as needed): headache 11% to 15%, upset stomach 8% to 10%, back pain 5% to 6%, muscle aches, stuffy nose and flushing about 3%.
- Back pain and muscle aches usually start 12 to 24 hours after a dose and fade within 48 hours, per the label.
- Viagra and Cialis together: the Cialis label says combining it with other PDE5 inhibitors has not been studied and tells patients not to do it. Ask your clinician.
- If you might need nitroglycerin for chest pain, the label says at least 48 hours must pass after your last Cialis dose.

01 · Generic tadalafil from about $20 or brand Cialis, no subscription
Best for As-Needed Tadalafil
HealthyMale
A long-running US men's-health pharmacy whose new-customer intro pricing on FDA-approved generics is the nearest legitimate equivalent to a free sample.
Clinical oversight: Verifiable accreditation (VIPPS pharmacies, LegitScript, HIPAA) and unusually transparent itemized pricing; intro offers rotate through its promotional channels, so confirm the current one on the site before you order.
HealthyMale is one of the longest-operating men's-health telehealth pharmacies in the US, reporting operations since 1998 and pairing US-licensed physicians with VIPPS-certified pharmacies. It matters for this page for one reason: its published pricing and rotating new-customer offers get closer to "free sample" territory than anything else we could verify in this category.
What the intro offer looks like. Through its promotional and affiliate channels, HealthyMale has advertised introductory bundles for first-time customers in the style of 20 generic ED pills for $39. That works out to roughly two dollars a dose for FDA-approved generic medication, prescribed by a licensed physician, from an accredited pharmacy. Intro offers rotate and terms change, so treat that as an example of the style of offer rather than a quote, and verify the current deal on the HealthyMale site.
Access runs through a phone consultation with a US-licensed physician, and a prescription is issued only at the clinician's discretion, which is exactly what a legitimate offer should require. For trust signals, the site displays LegitScript verification, HIPAA compliance, and VIPPS-certified pharmacies.

What we like
- Advertised new-customer intro bundles, the closest verifiable thing to a free ED sample
- FDA-approved generic sildenafil and tadalafil from ~$20 at regular prices
- Free doctor consultation advertised with purchase
- No forced subscription; VIPPS, LegitScript, and HIPAA trust signals
Worth noting
- No literally free product, a consult and purchase are still required
- Intro offers rotate, the exact bundle changes, verify before ordering
- Phone consult rather than on-demand video
Who should buy it: Adults 18+ who came looking for a free sample and want the legitimate near-equivalent: a low-cost first order of FDA-approved generics from an accredited pharmacy, with an advertised free consult and no subscription commitment.
What we don't like: Intro offers rotate and are not always identical, so the exact bundle you see may differ from what was advertised elsewhere; the consult is by phone rather than video.
Bottom line: If you searched for free ED samples, this is the offer to check first: HealthyMale has advertised introductory bundles for new customers in the style of 20 generic ED pills for $39, pairs them with an advertised free physician consultation with purchase, and sells FDA-approved generics from around $20 with no subscription. Verify the current intro offer on its site, promotions change.
02 · Lowest per-tablet price on FDA-approved generic tadalafil
Lowest Per-Tablet PriceNu Image Medical ED Tablets
A telehealth clinic founded in 2004, online since 2008 and available in all 50 states, that lists FDA-approved generic sildenafil and tadalafil tablets alongside compounded combination options.
Clinical oversight: Per-tablet prices published on the provider's own men's sexual performance page, with each product labeled FDA-approved or compounded. Every order is subject to a medical consultation and ships only if a licensed U.S. provider prescribes it.
Nu Image Medical is a telehealth clinic that says it was founded in 2004, has operated online since 2008 and is available in all 50 states. Its men's sexual performance line lists four options (all provider-published): a generic sildenafil tablet as low as $3 per tablet and a generic tadalafil tablet as low as $5 per tablet, both FDA-approved and both priced on a 12-month supply; a compounded sildenafil plus tadalafil rapid-dissolve tablet as low as $4 per tablet on a 5-month supply; and the compounded Mt. Everest program (tadalafil, oxytocin and PT-141) from $4 per tablet on a 5-month supply.
You share your goals online, a licensed U.S. provider reviews them, and medication ships discreetly only if it is prescribed.

What we like
- Lowest published per-tablet price we track on FDA-approved generics
- Each product labeled FDA-approved or compounded
- Available in all 50 states
Worth noting
- Lowest prices require a 12-month supply
- Combination and Mt. Everest products are compounded, not FDA-approved
Who should buy it: Adults 18+ who want an FDA-approved generic tablet at a low per-tablet price and are comfortable buying a multi-month supply.
What we don't like: The lowest prices depend on a 12-month supply, so the up-front cost is higher than a one-month order, and the combination products are compounded rather than FDA-approved.
Bottom line: If you want the FDA-approved generic tablet and are fine buying a longer supply, Nu Image lists the lowest per-tablet prices we track: sildenafil as low as $3 and tadalafil as low as $5. The low figures are tied to a 12-month supply, so check the up-front total before you pay.
03 · Cheapest published daily tadalafil plan (compounded)
Lowest Daily Plan PriceShed Steady (daily tadalafil)
A 5-minute online intake for a daily tadalafil 5mg tablet listed from $49.67 a month.
Clinical oversight: Prices and ingredients published on the provider's product page, which also states the tablets are compounded and not FDA-approved. A licensed provider reviews the intake before anything is prescribed.
Shed's men's sexual health line has two products: Steady, a once-daily tadalafil 5mg tablet from $49.67 per month, and Rise, an on-demand tablet combining apomorphine, tadalafil and vardenafil from $66.33 per month, taken 15 to 60 minutes before sex (provider-published). Apomorphine is FDA-approved for Parkinson's disease "off" episodes, not for ED. You complete a roughly 5-minute online intake, and a licensed provider decides whether to prescribe.

What we like
- Lowest published daily tadalafil price we track

Worth noting
- Compounded, not FDA-approved
- Check plan length behind the monthly price
Who should buy it: Adults 18+ who want a daily tadalafil routine at the lowest published monthly price and are comfortable with a compounded tablet.
What we don't like: Compounded rather than FDA-approved generic tablets, and the lowest monthly figure may assume a longer plan, so check the billing terms.
Bottom line: Shed publishes the lowest monthly price we track for a daily tadalafil plan, from $49.67 a month for 5mg. The trade-off is disclosed on its own page: both its ED tablets are compounded, not FDA-approved. A good fit if daily dosing is what a clinician recommends and price matters most.
04 · Flat monthly price for tadalafil capsules
Lowest Monthly PriceBayan Health ED Capsules
A telehealth clinic that lists sildenafil or tadalafil capsules at $49 a month, the lowest monthly figure we found.
Clinical oversight: Price published on the provider's own men's sexual health page; prescribing happens after an online intake reviewed by a licensed clinician. Capsules are a pharmacy-prepared format, so ask whether yours are compounded.
Bayan Health is a telehealth clinic whose men's sexual health line lists sildenafil or tadalafil capsules at $49 per month each on a 3-month subscription, alongside a combination "performance stack" (all provider-published). You complete an online intake, a licensed clinician reviews it, and medication ships only if it is prescribed.

- Consult

- Subscription
- Yes, 3-month plan
What we like
- Lowest published monthly price we track
- Fully online intake
- Also offers the other PDE5 option if a clinician prefers it
Worth noting
- 3-month subscription, not a one-time trial
- Dose per capsule not listed on the category page
- Capsule format may be compounded, confirm before ordering
Who should buy it: Adults 18+ who want a flat monthly price and are comfortable committing to a 3-month plan.
What we don't like: The $49 figure is tied to a 3-month subscription, the page does not list the dose per capsule, and capsules may be compounded rather than the FDA-approved generic tablet.
Bottom line: If you want a predictable monthly cost rather than a per-pill price, Bayan lists sildenafil or tadalafil capsules at $49 a month on a 3-month plan. It is not a sample, and the plan length matters, but it is the lowest monthly number on a provider's own page among the options we track.
05 · Video visit to settle dose and daily vs as-needed, then fill anywhere
Best Single-Visit OptionDrHouse
An on-demand video telehealth platform where a single visit, possibly covered by insurance, gets you a real prescription you can fill cheaply at any pharmacy.
Clinical oversight: LegitScript-approved and HIPAA-compliant with stated 50-state availability and a published visit price; the medication cost depends on the drug and pharmacy you pick, so the total is in your control but not fixed up front.
DrHouse approaches the sample question from the other side: instead of bundling medication, it makes the clinical visit itself fast and low-commitment. It is an on-demand telehealth platform, LegitScript-approved and HIPAA-compliant, available in all 50 states, where the company states a provider can join a video visit in roughly 15 minutes.
Why that matters for sample hunters. The ED visit is listed at $129 or your insurance copay, and DrHouse states insurance may cover the visit. When clinically appropriate, the physician e-prescribes sildenafil, tadalafil, vardenafil, avanafil, or alprostadil to the pharmacy of your choice, where the medication is priced separately. Generic sildenafil and tadalafil are inexpensive at many US pharmacies, especially with discount programs, so a copay visit plus a small generic fill can be one of the cheapest compliant ways to start.
DrHouse does not prescribe controlled substances, and a prescription is never guaranteed, the clinician decides. If you want an all-in bundle with medication included, HealthyMale's model will feel simpler; if you want clinical speed and pharmacy flexibility, DrHouse is the one.
- Status

- Consult
- On-demand video visit (~15 min), all 50 states

- Subscription
- None
- Credentials
- LegitScript-approved, HIPAA-compliant
What we like
- One-time visit, no subscription or auto-ship
- Insurance may cover the visit, potentially reducing it to a copay
- E-prescribes to any pharmacy, so you can shop the lowest generic price
- Fast: provider joins by video in about 15 minutes, per the company
Worth noting
- Medication priced separately, no bundled total
- Visit fee applies regardless of the prescribing decision
- No physical product ever ships from DrHouse itself
Who should buy it: Adults 18+ who want a one-time, no-subscription evaluation from a licensed physician, would like the chance to put insurance toward the visit, and are comfortable filling a generic prescription at their own pharmacy.
What we don't like: You have to price the medication yourself at the pharmacy, so there is no single all-in number to compare, and the visit fee applies even if the clinician decides medication is not appropriate.
Bottom line: The smart pick if your real question behind 'free samples' is 'what would this cost me to just try, once': one video visit at $129 or your copay, no subscription, and an e-prescription you can fill as a cheap generic at whatever pharmacy prices it lowest.
How long does Cialis last?
An as-needed dose of Cialis lasts up to 36 hours. The FDA label says tadalafil improved erectile function compared with placebo up to 36 hours after dosing. That is why tadalafil is often called the weekend pill.
"Lasts" means the window in which sexual stimulation can produce an erection more easily. It does not mean a constant erection, and Cialis does nothing without arousal. An erection that lasts more than 4 hours is a medical emergency.
- Cialis as needed (5 to 20 mg): effect up to 36 hours.
- Cialis once daily (2.5 to 5 mg): taken every day at about the same time, so there is no dosing window to plan around.
- Sildenafil (Viagra) and vardenafil (Levitra): short-acting, with half-lives of about 4 hours and 4 to 5 hours.
Which schedule fits you is covered in our daily vs as-needed tadalafil guide.
Cialis half-life
Tadalafil's mean terminal half-life is 17.5 hours in healthy men, according to the CIALIS label. Half-life is the time the body takes to clear half of a dose. A long half-life is what produces the long window, and it is what makes a low daily dose possible: blood levels build to a steady state rather than spiking and dropping.
The label also notes that exposure is higher in older men and in men with reduced kidney function, which is why those groups get lower maximum doses. For example, men with severe kidney impairment or on dialysis are limited to 5 mg as needed, no more than once every 72 hours.
How long does Cialis stay in your system?
Most of a single dose is cleared within about 3 to 4 days. That is a general pharmacology rule of thumb: a drug is largely eliminated after about five half-lives, and five times 17.5 hours is roughly 88 hours. The label does not give a single clearance time, and it varies with age, liver and kidney function and other drugs you take.
This matters for two reasons. Side effects like back pain can last into the next day. And interactions can happen long after you took the pill, which is why the label's nitrate rule uses a 48-hour wait.
How long does Cialis take to work?
Tadalafil reaches peak blood levels between 30 minutes and 6 hours after a dose, with a median of 2 hours, per the label. Food does not change how it is absorbed, so it can be taken with or without a meal. The label simply says to take as-needed Cialis before anticipated sexual activity, without a fixed lead time. Many men take it a few hours ahead, and because the window is so long, timing is less critical than with sildenafil.
For starting doses, strengths and the rules for older men and kidney or liver problems, see our tadalafil dosage guide.
Cialis side effects
The most common Cialis side effects are headache, upset stomach, back pain and muscle aches. In the label's placebo-controlled studies of as-needed use, the rates were:
- Headache: 11% at 10 mg, 15% at 20 mg (5% on placebo)
- Dyspepsia (indigestion): 8% at 10 mg, 10% at 20 mg (1% on placebo)
- Back pain: 5% at 10 mg, 6% at 20 mg (3% on placebo)
- Myalgia (muscle aches): 4% at 10 mg, 3% at 20 mg
- Nasal congestion: 3%
- Flushing: 3%
- Pain in a limb: 3%
Back pain and muscle aches are tadalafil's signature side effect. The label says they generally started 12 to 24 hours after dosing and typically resolved within 48 hours, felt as a diffuse ache in the lower back, buttocks or thighs. About 0.5% of men stopped as-needed Cialis because of them.
Get emergency care for an erection lasting more than 4 hours, sudden vision loss in one or both eyes, sudden hearing loss or ringing in the ears, chest pain, or fainting. If side effects persist, a clinician can lower the dose or switch to the daily low dose.
Can you take Viagra and Cialis together?
Not on your own. The CIALIS label says the safety and effectiveness of combining Cialis with other PDE5 inhibitors or ED treatments have not been studied, and it tells patients not to take Cialis with other PDE5 inhibitors. Viagra (sildenafil) is a PDE5 inhibitor, so the label's answer is no.
Stacking two drugs that both lower blood pressure raises the risk of dizziness, fainting and side effects, and because tadalafil lingers for days, a Viagra taken "the next day" can still overlap with it. If one drug is not working, the safer move is to ask a clinician about a dose change or a switch.
Cialis and alcohol
The label warns that substantial alcohol, 5 or more units, with Cialis can lower blood pressure and cause dizziness, a faster heart rate or headache. A drink or two is a different situation from heavy drinking, but alcohol also blunts erections on its own. Because tadalafil stays active for a day or more, the caution applies to the whole window, not only the hour after the pill.
Who should not take Cialis
Do not take Cialis if you use any nitrate or a guanylate cyclase stimulator such as riociguat. The combination can cause a dangerous drop in blood pressure. If you develop chest pain after Cialis and a nitrate is medically necessary, the label says at least 48 hours should pass after your last dose, and only with close monitoring.
- Alpha-blockers and blood pressure drugs: combining them can lower blood pressure. See ED meds and blood pressure.
- Strong CYP3A4 inhibitors such as ketoconazole or ritonavir: the label caps as-needed Cialis at 10 mg no more than once every 72 hours, and daily Cialis at 2.5 mg.
- Heart disease: the label lists recent heart attack, unstable angina, recent stroke and uncontrolled blood pressure among the groups not studied. Sex itself has to be safe for your heart.
- Adcirca: tadalafil is also sold for pulmonary arterial hypertension. Do not take Cialis if you already take Adcirca.
Daily vs as-needed Cialis
As-needed Cialis gives a window of up to 36 hours per dose. Daily Cialis at 2.5 or 5 mg keeps a steady level so you do not plan around a pill. Daily dosing also treats urinary symptoms of an enlarged prostate (BPH), which the label covers. The trade-off is cost and pill count: about 30 tablets a month. We cover the full decision in daily vs as-needed tadalafil, and price it out in how much generic Cialis costs.

Questions, answered
How long does Cialis last?
Up to 36 hours after an as-needed dose. The FDA label says tadalafil improved erectile function compared with placebo for up to 36 hours. Sexual stimulation is still needed, and an erection over 4 hours needs emergency care.
What is the half-life of Cialis?
The CIALIS label gives a mean terminal half-life of 17.5 hours in healthy men. Older men and men with reduced kidney function clear it more slowly.
How long does Cialis stay in your system?
Most of a dose is cleared in about 3 to 4 days, based on the rule that a drug is largely gone after about five half-lives. It varies by person, which is why the nitrate wait is 48 hours.
How long does it take for Cialis to kick in?
Peak blood levels arrive 30 minutes to 6 hours after a dose, median 2 hours, and food does not change that. The label says to take it before anticipated sexual activity without a fixed lead time.
What are the most common Cialis side effects?
Headache (11% to 15%), indigestion (8% to 10%), back pain (5% to 6%), muscle aches, stuffy nose and flushing (about 3%), per the label's as-needed studies at 10 mg and 20 mg.
Why does Cialis cause back pain?
The label does not give a cause, but it describes the pattern: a diffuse ache in the lower back, buttocks or thighs that starts 12 to 24 hours after a dose and usually resolves within 48 hours. Tests showed no underlying muscle or kidney damage.
Can you take Viagra and Cialis together?
Not without a clinician. The Cialis label says combinations with other PDE5 inhibitors have not been studied and tells patients not to take them together. Some compounded tablets combine both, but those are not FDA-approved and are prescribed case by case.
Can I drink alcohol with Cialis?
The label warns that 5 or more units of alcohol with Cialis can lower blood pressure and cause dizziness, a faster heartbeat or headache. Moderate drinking is a different situation, but ask your clinician.
Does Cialis last longer than Viagra?
Yes. Tadalafil works up to 36 hours and has a 17.5-hour half-life. Sildenafil is short-acting, with a half-life of about 4 hours, and is taken 30 minutes to 4 hours before sex.
